Assignment and Code of Conduct forms.
2) Apply for a passport if needed. Please do this early.
3) Send in fee money.
$750.00 USD made out to FOMMM.
4) Send us a photo
copy of credentials/licenses.
5) Get immunizations.
Yellow fever shots are required to
get into most places FOMMM goes. Hepatitis shots are
recommended.
6) Purchase plane
tickets. (Through us)
7) Apply for a visa
not more than 3 months before
we go. Visa requirements are different for most countries.
